Our practice is no longer an in-network provider with EyeMed Vision Care.
Patients with EyeMed insurance are welcome to continue care with us on a private-pay basis.
Itemized receipts can be provided upon request for possible out-of-network submission and FSA or HSA reimbursements.
If you prefer to receive care from an EyeMed in-network provider, we will provide you with
information upon request.
